Instructions to access the fillable form:
Download the form to your computer or smart device. Open the form with the app - Acrobat Reader. The Totals are automatically filled in EXCEPT for the Grand Final Total on the last page. Line 6 - Total Income MINUS Line 11 Total Expenses are automatically calculated from information previously given. Please manually subtract these two totals and insert the Final Total. This amount should reflect the bank statement total as of 1 July of this year.
Navigation of the form is as follows:
Income are Lines 1 - 5
Line 6 is the Total Income
Expenses are Lines 7 - 10 (these are in a Table format)
Line 11 is the Total Expenses
You must manually calculate you Final Total which will be separate from the last total of Other Assets as CD's, etc...
Either President or Treasurer can Sign the form -
Download, fill out, save then finally send to BVA National Headquarters for further processing. Do not forget to include with Report, All Bank Statements. If your Regional Group has a 501 C (3) include Certificate of 501 C (3) status with Regional Group Tax Exempt Number.
In order to be in "Good Standing", Report Must be at National Headquarters no later than 30 September this year.
